What is the path and filename that mcaffee reports? What OS are you running?

Could be that the file is in system restore. That would account for nothing being able to remove it.

If the path is c:\system volume information ....... then to remove it you must disable system restore, by right clicking on "my computer"on the desktop, and choosing properties.
On the system restore tab, check the box turn off system restore on all drives. click OK, and reboot.
That will remove the infected restore points.
Then repeat the above process, this time, removing the checkmark.
Then set a clean restore point from help & support>undo changes to your computer using system restore.
